Adam Salky‘s I Smile Back, which premiered at Sundance earlier this year, will be stopping by Toronto International Film Festival and for the occasion, we have the first trailer. The grim, addiction drama stars Sarah Silverman in a rare leading turn as Laney, a suburban mother and housewife that suffers from a drug-fueled depression. The film also co-stars Josh Charles as Laney’s attentive husband, Thomas Sadoski, Mia Barron, Terry Kinney, and Chris Sarandon.

We said in our review, “The central character in I Smile Back is Addiction, not Laney. Despite the strong turn by Silverman, it too often feels like a macro representation of the Disease. We know Laney only as an addict, and not much else. She’s a good mother in spots and a good wife as well, but only for the small amount of time needed tease at hope, only to rip it away. At a running time of 85 minutes, we learn too little about Laney and too much about her addiction.”

Check out the trailer below ahead of an October release via Broad Green.

I Smile Back explores the life of Laney (Sarah Silverman), an attractive, intelligent suburban wife and devoted mother of two adorable children. She has the perfect husband (Josh Charles), a pristine house, and a shiny SUV for carting the children to their next activity. However, just beneath the façade lie depression and disillusionment that send her careening into a secret world of reckless compulsion. Only very real danger will force her to face the painful root of her destructiveness and its crumbling effect on those she loves.

I Smile Back opens on October 23rd.
